This Registration has three parts.
1. Fill out the Registration Form  below.

2. Calculate your fees.
2. Print the Waiver (see Step 3), sign and mail in with the cheques.

Summerland Minor Hockey Publicity Waiver

I hereby give my consent to the Summerland Minor Hockey Association to use my image in the form of a photograph, videotape likeness or any other recording or reproductions for promotional purposes, without payment of any fee or charge, in the following areas (please indicate):

Please Note: If the player was enrolled in a different association last year,

a Declaration of Residence Form must be filled out. The transfer of player cannot be completed until this form is received.

 

Though players are eligible to practice after the transfer has been requested, but before it has been approved, players cannot participate in any games until the transfer has been completed at the OMAHA and BCHA level.

HOCKEY SKILLS CAMP 

Participants for the Hockey Skills Camp must be registered in SMHA to be eligible to participate in this Camp (for insurance purposes).

Do you wish to enroll in the Hockey Skills Camp to take place TBA.

STEP 2 - CALCULATE YOUR FEES

* Registration date is June 15 - All mail containing registration forms and payments must be post-marked by June 15th or earlier. 

ALL FEES MUST BE PAID IN FULL AT THE TIME OF REGISTRATION

EARLY REGISTRATION CHEQUES MAY BE POST DATED TO AUGUST 1, 2010

Additional post-dated cheques required with the registration:

1.  Post-dated cheque for January 1, 2011 for $25 as a bottle drive/fund raising deposit.  This cheque will be returned to you upon confirmation of your participation in the bottle drive/fundraising event.

2.  A uniform deposit fee of $50 per player will be made by post-dated cheque for April 30, 2011.  This cheque will be returned if the jersey is returned to the team manager before this date.

Registration form and payment in full must be received by June 15, 2010 with cheque post-dated to August 1, 2010 in order to qualify for the AGM  discount and not be charged a late Registration fee.
