Coaching Application Process:
1. Please fill out form to the best of your ability and submit to the Head Coach of SMHA.
- If process is not followed correctly, or all requested documents are not received, application will not be considered
2. Please submit this completed form*, with all requested fields filled out, and then email requested practice plans.
SMHA thanks you for your interest at this time. Only successful candidates will be contacted for a follow up interview.
*If you are applying for multiple coaching assignments, please only submit one application; you are responsible for sending in age group specific practice plans for each level interested in.
